We are very proud of this most important and needed service, and it is our pleasure to offer you a safe and efficient means of transportation to help you live independently and to remain an active and vital part of our community!
Family Foundation Group provides curb-to-curb service to seniors, age 55 and older, who reside within the boundaries of these Cook County South Suburbs. Transportation to medical appointments is limited. In an effort to best serve all of our passengers, we ask that you keep the following guidelines in mind while using this service.
HOURS
Transportation service is available by appointment only between the hours of 9:00 a.m. to 3:00 p.m.
Monday through Friday only.
FEES
$5.00 (per person) per unit of service (one-way) Per 5 Miles from Your Home to Your Destination
Payment envelopes will be provided for your use. We ask that you give the envelope, with your full payment enclosed, to the first driver who will be driving you to your destination. Please provide the exact amount due for transportation services at the time.
RESERVATIONS
Can be made by calling (888) 359-8446 Monday through Friday from 9:00 a.m. to 3:00 p.m.
When calling to schedule a ride, be ready to provide the following passenger information:
1. ID number, name, address, and phone number
2. Number of persons traveling
3. Date of the trip
4. The exact street address (and building entrance) of each location, pick-up and drop-off
5. For doctor’s visits give appointment time, length of visit, doctor name, address, and phone.
The request for transportation will be processed and you will receive a call back to confirm your trip date and actual pick-up and return times. When scheduling, the transportation coordinator will try to fit your ride in as near to the requested time as possible. However, it is helpful if times can be somewhat flexible. Lunch hour appointments are more difficult to arrange.
All requests for transportation can be scheduled up to one week in advance. You may schedule unlimited rides for medical appointments, however, two non-medical rides per week may be scheduled. Requests for transportation to medical providers have always been our top priority.
Please note, although we try to accommodate every request, there may be times which we cannot always provide what is requested.
If you have special needs, please inform the Transportation Department when scheduling your ride. All clients must be independently mobile. Persons not entirely mobile will need to provide their own personal care attendant for assistance.
Drivers are not authorized to make changes in trip times or pick-up or drop-off locations. All requests for changes must be scheduled with the office in advance.
PLEASE BE READY!
Watch for your vehicle to arrive. As much as our drivers try to be on time, traffic conditions can alter their schedules. Please be flexible and allow the drivers to be fifteen minutes early or late. It is important for you to be prompt!
CANCELLATIONS
Always remember to cancel or reschedule your ride if you cannot make your appointed pick-up time. Please check your calendar carefully before scheduling a ride.
LIMIT PACKAGES
Please limit your packages to what you can carry. Our drivers are here to serve you, however they are not allowed beyond your doorstep. Please do not ask the drivers to make exceptions to these rules.
PASSENGER SAFETY
Your safety is important, so we ask you to please wear your seat belt. Smoking, drinking and eating are prohibited in the vehicles.
Family Foundation Group reserves the right to cancel transportation service on days severe weather conditions exist.
